You need to submit all of the information into this form at one time. You can not add information, save the form and return to add the rest. Please, therefore, start the process once you have all the information finalised and in hand.

Guidance for profile image
- When selecting an image of yourself to represent you in Artangel's public communications about your contribution, please select one of at least 3000 pixels wide and upload it at the highest resolution you have available.
- We can not accept smaller or low resolution images as they will not be suitable for print and can look degraded on some screens at the size we intend to use them at.
- If you have a selection we can choose from, even better! Please just zip them together into one file before uploading. If there are different captions for each, please include these as a text file within the zipped file.
- Please ensure you have the permission from all rights holders and anyone appearing in the image for Artangel to make use of this image in print and online, universally, in perpetuity.
- The image does not have to be a clear head and shoulders image, please choose whatever you feel most comfortable to have representing you.
